The mission at Noblequest Health Foundation is to build the first person-centered medical community in Southern California. Our integrated whole-person approach puts you and your family in the center of our collaborative network.
Our Vision //
The vision of Noblequest is to become your partner of choice in bridging the gap between your healthcare needs including psychosocial, wellness, and community resources.
Noblequest Health Foundation presently provides primary care, mental health services, case management, community support, housing navigation, and placement services. Our group of social workers will assist you and your family in welfare application, Family-PACT, Medi-Cal application, In Home Service Support, CalFresh, and many more. We offer basic health services to those who are uninsured, unemployed, and undocumented. Our network of providers agreed to provide services following our discounted fee schedule. Please ask the office staff for the "Discount Sliding Scale Application."
To ensure that we directly support the population, often overlooked by traditional healthcare providers, we also offer our members experiencing homelessness with housing placement, rapid re-housing, and recuperative care services.
Noblequest Health Foundation has been providing services to populations in need since 2013. This community-based approach has allowed us to continuously expand our team and meet the needs of the communities we serve. We currently have a staff of 100 people.
